Ground rules for Using CareShare-CMHI.org

• With the family’s consent, any family or community partner, in collaboration with their medical home (primary care practice staff) can initiate a Care Share Page for a child.

• It is the child/youth’s parents or guardians who grant access privileges to their child’s page and the duration of access to information.

• All information is shared in the spirit of collaboration helping to contribute to a consistent approach to each child from a well informed team.

• CareShare is not meant to replace typical medical information exchange, the electronic medical record, clinician and other professional discourse, nor the necessary face to face contact to develop and maintain a relationship.

Risk of Using CareShare-CMHI.org

Transmitting information using CareShare-CMHI.org has a number of risks that all partners (patient and parent/guardian, clinicians, educators, etc) should consider before using. Information could be saved, copied, circulated and therefore shared with unintended recipients.

Conditions for the Use of CareShare-CMHI.org

I will use all reasonable means to maintain security and confidentiality of CareShare content. I am not liable for improper disclosure of confidential information that is not caused by my intentional misconduct.

Patient and parent/guardian, clinicians, educators, and their staff cannot guarantee that any particular communication will be read and responded to within any particular period of time but will make every effort to be timely and prompt.

• The patient (parent/guardian) should schedule an appointment if the issue is too complex or sensitive to discuss via CareShare.

• It is the patient’s (parent/guardian) responsibility to follow-up and/or schedule an appointment if warranted.

• It is the primary care staff and community partners’ responsibility to suggest an appointment for follow up as deemed appropriate.
