About CareShare

CareShare has been designed by the Center for Medical Home Improvement (CMHI) to support communication and collaboration among families, caregivers at their medical home, and key community partners. Currently you need to be one of the practices or a participant in the Beyond the Medical Home initiative in order to use CareShare. It is a new tool and is undergoing testing.  Ask your doctor or  care coordinator to help you get started and/or contact Leah Reed at CMHI (leah.reed@crotchedmountain.org) 603-547-3311 Ext 511.
